Transfer or Concurrent Student Application Procedure

Obtain the correct form from the Graduate College website in the Forms section:

• Request to Transfer from One Major/Program/Department to Another -- http://www.grad-college.iastate.edu/forms/files/MajorTransfer.doc
• Request to Pursue Two Concurrent Graduate Degrees -- http://www.grad-college.iastate.edu/forms/files/ConcurrentDegrees.doc

Complete Section I and take to your current department to start the process. Ask them to send the form along with the following information to Mathematics:

• TOEFL score (if applicable)
• GRE scores
• Graduate Admissions Evaluation w/ application and transcripts
• Previous letters of recommendation
